  • The PROMIS-29 Profile form is available in the Assessments module.

  • The form contains 29 mandatory questions grouped across multiple health domains.

  • Automatic calculation of raw scores for each domain during form completion.

  • Automatic generation and storage of scaled T-scores upon form submission.

  • Each question and score is mapped to its corresponding LOINC code.

  • Completed forms and scores are saved in the patient record for future review.

How It Works 

  • Navigate to the patient chart and open the Assessments module.

  • Select the PROMIS-29 Profile form.

  • The form displays 29 required questions grouped by domain, including:

    • Physical Function

    • Anxiety

    • Depression

    • Fatigue

    • Sleep Disturbance

    • Ability to Participate in Social Roles and Activities

    • Pain Interference

  • The user must complete all questions before the form can be saved.

  • If any question is left unanswered, the system displays an alert requesting completion of all fields.

  • As responses are entered, raw scores are calculated automatically for each domain.

  • Upon saving the form, both raw scores and scaled T-scores are generated using a predefined scoring guide and stored in the patient record.

  • Scores and responses are displayed alongside the completed form for reference.
