Children who receive a fluoride varnish application.

Exclude patients who were in hospice care during the measurement year

For Denominator:

The patient of age 0-20 with a visit during the measurement year and one of the following encounter has been performed is considered for this measure.

1. Face-to-Face Interaction
2. Preventive Care – Established Office Visit, 0 to 17″
3.  Preventive Care- Initial Office Visit, 0 to 17″
4.  Preventive Care Services – Established Office Visit, 18 and Up”
5. Preventive Care Services-Initial Office Visit, 18 and Up”
6. Office Visit”
7. Clinical Oral Evaluation”

For Numerator:

To qualify for numerator, the patient has to have a procedure done for Flouride varnish application. To record the procedure, navigate to Patient > Clinical > Lab and Orders > Procedure Order.
