DENOMINATOR

Patients 65 years of age and older with a visit during the measurement period.

-DENOMINATOR EXCLUSION

Exclude patients who were in hospice care during the measurement year.

NUMERATOR

Patients who have ever received a pneumococcal vaccination.

APPLICATION WORKFLOW

For Denominator:
Patients 65 years of age and older with a visit during the measurement period are included in the denominator of this measure. To record a visit, go to Patient > Provider Note

  • History of Pneumococcal Vaccine (Patient > Clinical > Immunizations > Add History Dose)
  • Procedure for Pneumococcal Vaccine Administered (Patient > Clinical > Evaluations)
