What is Patient Demographics in Form A?
Patient > Registration > Profile > Demographics
The following patient information is populated on the ‘ACOG Report’:
- Name
- ID
- Birth Date
- Marital Status
- Address
- Phone
- Insurance
Users can enter the required fields on ‘Demographics’ screen. This information is then automatically printed on the ACOG Report.
