What is Medical History in Form A?
The Medication History section of the ACOG Report is divided into three categories.
- Clinical
- Diseases
- Social History
Clinical: This section records the clinical intake for any patient. Its component includes Allergies, Diagnoses, Complaints, Family History, Medical History, Medications etc.
Follow the workflow to add ‘Allergy’ or check ‘No Known Drug Allergy’ for Patient.
Clinical information of patient with no drug and latex allergies can be viewed on ACOG report.
- If there is no drug and latex allergies or reactions and the ‘No Known Drug Allergy’ checkbox is selected, the negative symbol ‘O’ and ‘NKDA’ will appear in the part A of Medical History.
- If the patient has latex allergies but no drug allergies and the ‘No Knows Drug Allergy’ checkbox is selected, the positive symbol ‘+’ and ‘NKDA’ appears in the details for part A along with “Latex”.
- If a drug allergy is reported for the patient, the positive symbol ‘+’ and the drug appears in the details for part A.

Diseases: This section maintains a record of all the diseases that have been added for a patient. These diseases can be added by:
Navigate to ‘Medical History’. Here, search for the desired disease and enter its details. User can add a disease into an already existing category or even make a new category by clicking on Add to Library.
When user clicks on ‘Add to Library’, a pop up appears. Add disease name and disease category. The diseases on ACOG Report are populated from here.
Social History: This section maintains a record of social history like smoking, drinking etc.
This can be added by navigating to ‘Social History’ section.
