How to Add a HCFA Form?
Settings > Reports > HCFA > HCFA Form Selection
Follow the workflow above to land on the ‘HCFA Form Selection’ screen.

Click ‘+Add’ button to display the ‘Add’ screen.
Select the file type from the drop down option and add description.
Add Form
Click ‘Save’ to view the records in a tabular form.
