What is a Care Plan?
Patient > Clinical > Care Plan
Follow the above mentioned workflow to land on ‘Care Plan’ tab.
However this component needs to be enabled from ‘Practice User’ permissions. To view how, click here.
Click ‘Add’ to add a ‘Desired Outcome’, ‘Priority Goal’ or a ‘General Goal’.
To record ‘Desired Outcomes’ of the respective ‘Care Plan’, click ‘Add’ adjacent to it and click ‘Save’ after writing down the desired outcome in ‘Outcome’ field.
The goals vital to patient’s health are recorded under ‘Priority Goals’ section.
Click ‘Add’, enter ‘Goal’ and ‘Description’ and click ‘Save’
Similarly, the goals that are not priority are recorded under ‘General Goals’.
User can record an ‘Evaluation’, a ‘Barrier’ or an ‘Intervention’ by clicking the menu (
) icon that appears next to the recorded goal. Fill in the required details and click ‘Save’. While recording an evaluation, user can record the progress from the ‘% Progress’ dropdown.
When a goal is listed, the percentage progress is reflected via a green bar.
User can move a general goal to priority by selecting ‘Mark Priority’ option from the carrot menu.
User can also change the status of the goal to ‘Not started’, ‘In Progress’, ‘Completed’, ‘On Hold’ and ‘Inactive’ by selecting the desired option from dropdown adjacent to the carrot menu.
User can sort the Clinical Module by dragging the components to the desired position in the list. To view how, click here.
‘Audit Trail’ is maintained related to changes made in ‘Care Plan’ component. To view more, click here.
The smart phrases can be used in care plan.
This adds convenience to user experience as they don’t have to write lengthy descriptions.
In order to use smart phrase, write full stop followed by smart phrase, for example, ‘.Cardiac’ in ‘Outcome’ text field and press ‘Ctrl + Space’.
The system populates the description in the text field. Click ‘Save’.
Similarly, the smart phrase can be typed in text field of ‘Goal’ of ‘Priority goals’.
Click ‘Save’ after the description populates in the text field.
