Exclude patients whose hospice care overlaps the measurement period.

For Denominator:

The patient of age 0-20 with a visit during the measurement year and one of the following encounter has been performed is considered for this measure.

1. Preventive Care – Established Office Visit, 0 to 17″
2.  Preventive Care- Initial Office Visit, 0 to 17″
3.  Preventive Care Services – Established Office Visit, 18 and Up”
4. Preventive Care Services-Initial Office Visit, 18 and Up”
5. Office Visit”
6. Clinical Oral Evaluation”

For Numerator:

To qualify for numerator, the patient has to have a procedure done for Flouride varnish application. To record the procedure, navigate to Patient > Clinical > Lab and Orders > Procedure Order.
