Number of patients, regardless of age, who were referred by one provider to another provider, and who had a visit during the measurement period

NUMERATOR

Number of patients with a referral, for which the referring provider received a report from the provider to whom the patient was referred

For Denominator:

A patient, regardless of his age, who is referred by one provider to another provider, and who had a visit during the measurement period is considered for this measure.

For Numerator

Any patient, from denominator or initial population, for whom the referring provider received a consultant report will be included in the numerator. The consultant report should contain the outgoing referral ID and must be received on a date after the referring provider sent the referral.
