– DENOMINATOR:
Patient visits for patients that are 17 years of age or older at the start of the measurement period during which a new diagnosis of MDD, single or recurrent episode, was identified. A qualifying MDD encounter in this case can include any of the following:
Note: The major depressive disorder (MDD) diagnosis must take placing during one of these visits. – NUMERATOR: Patient visits during which a new diagnosis of MDD, single or recurrent episode, was identified and a suicide risk assessment was completed during the visit. Note: The suicide risk assessment must happen during an encounter for Major Depressive Disorder. |

For Numerator: To document the risk assessment, navigate to Patient > Provider Note > Evaluations. Click ‘Add’ and search for an intervention. Select ‘Performed’ from the ‘Status’ dropdown. Then click ‘Save & Accept’ or ‘Sign & Accept’. |
