Patients 65 years and older who had a visit during the measurement period – NUMERATOR 1 Patients with an order for at least one high-risk medication during the measurement period – NUMERATOR 2 Patients with at least two orders for the same high-risk medication during the measurement period – Denominator Exclusions Exclude patients whose hospice care overlaps the measurement period |

– APPLICATION WORKFLOW
For Denominator: The patient of age 65 and above with a visit during the measurement year is considered for this measure. To record this, navigate to Patient > Provider Note > eSuperbill.
For Numerator 1: On a visit during the measurement year, patient with an order for at least one high-risk medication during measurement period is included in the numerator. In order to record this, navigate to Patient > Provider Notes > Prescription.
For Numerator 2:
For Denominator Exclusions: Exclude patients whose hospice care overlaps the measurement period. To document this, navigate to Patient > Clinical > Evaluation.
