CMS156v8 – Use of High-Risk Medications in the Elderly

Use of High-Risk Medications in the Elderly CMS156v8 Percentage of patients 65 years of age and older who were ordered high-risk medications. Two rates are reported.

  1. Percentage of patients who were ordered at least one high-risk medication.
  2. Percentage of patients who were ordered at least two of the same high-risk medications.
– DENOMINATOR

Patients 65 years and older who had a visit during the measurement period

– NUMERATOR 1

Patients with an order for at least one high-risk medication during the measurement period

– NUMERATOR 2

Patients with at least two orders for the same high-risk medication during the measurement period

– Denominator Exclusions

Exclude patients whose hospice care overlaps the measurement period

– APPLICATION WORKFLOW

For Denominator:

The patient of age 65 and above with a visit during the measurement year is considered for this measure. To record this, navigate to Patient > Provider Note > eSuperbill.

For Numerator 1:

On a visit during the measurement year, patient with an order for at least one high-risk medication during measurement period is included in the numerator. In order to record this, navigate to Patient > Provider Notes > Prescription.

For Numerator 2:
On a visit during the measurement year, patients with at least two orders for the same high-risk medication during the measurement period is included in numerator 2. In order to record this, navigate to Patient > Provider Notes > Prescription.

For Denominator Exclusions:

Exclude patients whose hospice care overlaps the measurement period. To document this, navigate to Patient > Clinical > Evaluation.
