CMS156v5 – Use of High-Risk Medications in the Elderly

Use of High-Risk Medications in the Elderly CMS156v5 The percentage of patients 66 years of age and older who were ordered high-risk medications. Two rates are reported:

  • Percentage of patients who were ordered at least one high-risk medication.
  • Percentage of patients who were ordered at least two different high-risk medications
– DENOMINATOR

Patients of age 66 years and above, who had a visit during the measurement period.

– NUMERATOR 1

Patients with an order for at least one high-risk medication during the measurement period.

– NUMERATOR 2

Patients with an order for at least two different high-risk medications during the measurement period.

– Denominator Exclusions

Exclude patients who were in hospice care during the measurement year

– APPLICATION WORKFLOW

For Denominator:
The patient of age 65 and above with a visit during the measurement year is considered for this measure. To record this, navigate to Patient > Provider Note > eSuperbill 

For Numerator 1:

On a visit during the measurement year patient with an order for at least one high-risk medication that is to be taken for more than 90 days is included in the numerator. In order to record this, navigate to Patient > Clinical > Medications.

Medications

For Numerator 2:
On a visit during the measurement year, patient with an order for at least two different high-risk medications, is included in numerator 2. In order to record this, navigate to Patient > Clinical > Medications.

Medication

– Denominator Exclusions

Exclude patients who were in hospice care during the measurement year. To document this, navigate to Patient > Clinical > Evaluation
