– NUMERATOR – 1 Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period |
– NUMERATOR – 2 Patients who had counseling for nutrition during the measurement period |
– NUMERATOR – 3 Patients who had counseling for physical activity during the measurement period |
– DENOMINATOR EXCLUSIONS
Patients who have a diagnosis of pregnancy during the measurement period. |

– APPLICATION WORKFLOW
For Denominator: Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) during the measurement period are included in the denominator of this measure. For this measure, a patient visit can be documented by navigating to Patient > Provider Note > eSuperbill. For Numerator 1: Patients who had height, weight and body mass index (BMI) percentile recorded during the measurement period are included in numerator 1. These can be recorded by navigating to Patient > Clinical > Vitals.
For Numerator 2: Patients who had counseling for nutrition measurement period are included in numerator 2. To record counseling, navigate to Patient > Clinical > Evaluations. Following codes may be used:
For Numerator 3: Patients who had counseling for physical activity during a visit that occurs during the measurement period are included in numerator 3. To record this, navigate to Patient > Clinical > Evaluations. Following codes may be used:
For Denominator Exclusions: Patients who have a diagnosis of pregnancy or were in hospice care during the measurement period are included in denominator exclusion. To record diagnosis, navigate to Patient > Clinical > Diagnosis. To record hospice care, navigate to Patient > Clinical > Evaluations and use the below mentioned codes:
