For Denominator: For Numerator: The documentation of the first encounter takes place when one visit code is documented on the patient’s eSuperbill/ charge. Patients for whom an assessment of cognition is performed and the results reviewed at least once within a 12 month period are included in the numerator. Any one of the following must be documented:
