Falls: Screening for Future Fall Risk CMS139v5 Percentage of patients 65 years of age and older who were screened for future fall risk during the measurement period.
– DENOMINATOR

Patients of age 65 years and older with a visit during the measurement period.

– NUMERATOR

All patients that were screened for future fall risk at least once within the measurement period.

– DENOMINATOR EXCEPTIONS

Documentation of medical reason(s) for not screening for fall risk (e.g. patient is not ambulatory).

– APPLICATION WORKFLOW

For Denominator:
The patient of age 65 and above with a visit during the measurement year is considered for this measure. To record a visit, go to Patient > Provider Note.

First Visit

The documentation of the first encounter takes place when one visit code gets attached with the patient.

For Numerator:
If the patient is screened for Future Fall Risk at least once within the measurement period then he is included in the numerator of this measure. To record Falls Screening, navigate to Patient > Clinical > Evaluations.

Future Fall Risk – Screening

For Denominator Exception:
If the patient has a documentation of medical reason(s)such as, patient is not ambulatory, then he is considered in the denominator exception of this measure.
