All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period are included in Denominator. – NUMERATOR Patients who were screened for tobacco use at least once within 24 months AND who received tobacco cessation intervention if identified as a tobacco user. -DENOMINATOR EXCEPTION Documentation of medical reason(s) for not screening for tobacco use (eg, limited life expectancy, other medical reason). |

For Denominator: The documentation of the first encounter takes place when one visit code gets attached with the patient’s eSuperbill/ charge.For Numerator:
-Denominator Exception Documentation of medical reason(s) for not screening for tobacco use e.g. limited life expectancy. Limited life expectancy has a diagnosis code. R99. It needs to be added via diagnosis (Patient > Clinical > Diagnosis) with PFT turned off. |
