– DENOMINATOR

Patients 65 years of age and older at the start of the measurement period with a visit during the measurement period.

The qualifying visit can include any of the following:

  • Office Visit
  • Annual Wellness Visit
  • Preventive Care Services Established Office Visit, 18 and Up
  • Preventive Care Services Initial Office Visit, 18 and Up
  • Home Healthcare Services
  • Care Services in Long Term Residential Facility
  • Nursing Facility Visit
  • Discharge Services Nursing Facility
  • Online Assessments
  • Telephone Visits
  • Office or other outpatient visit for the evaluation and management of an established patient that may not require the presence of physician or other qualified healthcare professional 

– DENOMINATOR EXCLUSION

Patients who are in hospice care for any part of the measurement period.

Patients with anaphylaxis due to pneumococcal conjugate vaccine or polysaccharide vaccine any time before the end of the measurement period.

– NUMERATOR

Patients who received a pneumococcal vaccination on or after their 19th birthday and before the end of the measurement period.

APPLICATION WORKFLOW

For Denominator:

To record an encounter, navigate to Patient > Provider Note > Create Superbill. Under the ‘Procedure-CPTs’ heading, enter the relevant encounter code.

For Numerator:

  • To administer a vaccine, use the workflow Patient > Provider Note > Immunization. Click ‘+Dosage’ and select ‘Administer Vaccine’ in the pneumococcal vaccine category. In the vaccine search window, search the pneumococcal vaccine. Complete the vaccine details and click ‘Save’.

  • The document the administration of the pneumococcal vaccination as procedure, navigate to Patient > Provider Note > Procedures. Click ‘Add’ and search for the relevant procedure. Once done, click ‘Save’.

For Denominator Exclusion:

  • To document that a patient is receiving hospice care outside of a hospital or long-term care facility, navigate to Patient > Provider Note > Evaluations. Use the below mentioned codes to order or perform an intervention:
    • 385763009: Hospice Care (Regime/Therapy)
    • 385765002: Hospice Care Management (Procedure)

  • To document an inpatient encounter, navigate to Patient > Provider Note > Evaluations.
Example of Inpatient Encounter
  • To document a hospice care assessment, follow the workflow Patient > Provider Note > Evaluations. Click ‘Add’ and search for ‘Hospice Care [Minimum Data Set]’. Then select ‘Performed’ from the ‘Status’ dropdown and ‘Yes’ from the ‘Result’ field.

  • A hospice encounter can be documented through Patient > Provider Note > Evaluations or Patient > Provider Note > Create Superbill.
  • To document ‘Anaphylaxis caused by vaccine product containing Streptococcus pneumoniae antigen (disorder), navigate to Patient > Provider Note > Evaluations.
