CMS127v11 – Pneumococcal Vaccination Status for Older Adults

Patients 66 years of age and older at the start of the measurement period with a visit during the measurement period.

The qualifying visit can include any of the following:

• Office Visit
• Annual Wellness Visit
• Preventive Care Services Established Office Visit, 18 and Up
• Preventive Care Services Initial Office Visit, 18 and Up
• Home Healthcare Services
• Care Services in Long Term Residential Facility
• Nursing Facility Visit
• Discharge Services Nursing Facility
• Online Assessments
• Telephone Visits

– DENOMINATOR EXCLUSION

Patients who are in hospice care for any part of the measurement period.

– NUMERATOR

Patients who received a pneumococcal vaccination on or after their 60^th birthday and before the end of the measurement period.

For Denominator:

To record an encounter, navigate to Patient > Provider Notes > Create Superbill. Under the ‘Procedure-CPTs’ heading, enter the encounter code.

For Numerator:

• To document pneumococcal immunization, use the workflow Patient > Provider Notes > Immunization. Click ‘+Dosage’ and select ‘Administer Vaccine’ in the pneumococcal vaccine category. In the vaccine search window, search the pneumococcal vaccine. Complete the vaccine details and click ‘Save’.

• The document the administration of the pneumococcal vaccination as procedure, navigate to Patient > Provider Notes > Procedures. Click ‘Add’ and search for the relevant procedure. Once done, click ‘Save’.

  

For Denominator Exclusion:

To document a patient receiving hospice care, navigate to Patient > Provider Notes > Evaluations. Hospice Care can also be recorded by the workflow Patient > Provider Notes > Create Superbill. Under the ‘Procedure-CPTs’ heading, add the relevant hospice care code.
