Breast Cancer Screening CMS125v7  Percentage of women 50-74 years of age who had a mammogram to screen for breast cancer
DENOMINATOR

Women 51-74 years of age with a visit during the measurement period

DENOMINATOR EXCLUSION

Women who had a bilateral mastectomy or who have a history of a bilateral mastectomy or for whom there is evidence of a right and a left unilateral mastectomy.

Exclude patients whose hospice care overlaps the measurement period.

NUMERATOR

Women with one or more mammograms during the measurement period or the 15 months prior to the measurement period

APPLICATION WORKFLOW

For Denominator:
Women, 51-74 years of age, with a visit during the measurement period are included in the denominator of this measure. To record a visit, navigate to Patient > Provider Note > eSuperbill

For Numerator:

Women with one or more mammograms during the measurement period or 15 months prior to the measurement period is included in the numerator. To record a mammography, navigate to Patient > Clinical > Evaluations.

User can add Mammogram through eSuperbills (Patient > Provider Notes > eSuperbills) if HCPCS such as. G0202, G0203 and G0204 are used.

For Denominator Exclusions:

If the patient had a bilateral mastectomy or who have a history of a bilateral mastectomy or for whom there is evidence of a right and a left unilateral mastectomy, then she gets excluded from the denominator.

Also, exclude patients whose hospice care overlaps the measurement period, to record it navigate to Patient > Clinical  > Evaluation.

  • Right Mastectomy (Patient > Clinical > Diagnosis)

  • History Bilateral Mastectomy (Patient > Clinical > Diagnosis)
  • Bilateral Mastectomy Procedure (Patient > Clinical > Orders and Results > Procedures or Patient > Clinical  > Evaluation)
