How to Identify a Patient?

Patients are able to find their profile by using the search criteria on ‘Identify Yourself’ screen after login.
User can search the profile by DOB, Home Phone or Cell Phone and SSN.
After entering the DOB, user either need to enter Cell/Home number or last 4 digits of SSN. Click ‘Find my Profile’ button to see the patient profile information.
