How to Document Reason for not Recording Height and Weight?
Patient > Clinical > Vitals
You can document reason for not recording weight and height (BMI is automatically calculated by entering these values) by navigating to Patient > Clinical > Vitals. Enter the required vitals and click on Save. A pop up is displayed. Here you can select any reason from the drop down. When done click on Save.
NOTE: You can only document refusal by enabling checkboxes in My Preferences.
