Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents CMS155v5 Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported.

  • Percentage of patients with height, weight, and body mass index (BMI) percentile documentation
  • Percentage of patients with counseling for nutrition
  • Percentage of patients with counseling for physical activity
– INITIAL POPULATION

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period.

– DENOMINATOR – 1

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP)

– NUMERATOR –  1

Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period.

– DENOMINATOR – 2

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period.

– NUMERATOR – 2

Patients who had counseling for nutrition during a visit that occurs during the measurement period.

– DENOMINATOR –  3

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period.

– NUMERATOR –  3

Patients who had counseling for physical activity during a visit that occurs during the measurement period.

– DENOMINATOR EXCLUSIONS

Patients who have a diagnosis of pregnancy during the measurement period.
Exclude patients who were in hospice care during the measurement year.

– APPLICATION WORKFLOW

For Denominator:

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) during the measurement period are included in the denominator of this measure.

Outpatient Visit

For Numerator  1:

Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period. The following records must be added:

  1. BMI percentile (Patient > Clinical > Vitals)
  2. Height (Patient > Clinical > Vitals)
  3. Weight (Patient > Clinical > Vitals)

 

Vitals

For Numerator 2:

Patients who had counseling for nutrition during a visit that occurs during the measurement period are included in numerator stratum 2. To record counseling, navigate to Patient > Provider Note > eSuperbill.

Counseling for Nutrition

For Numerator  3:

Patients who had counseling for physical activity during a visit that occurs during the measurement period are included in numerator stratum 3. To record this, navigate to Patient > Clinical > Evaluations.

Denominator Exclusions:

Patients who have a diagnosis of pregnancy during the measurement period.
Exclude patients who were in hospice care during the measurement year.
