  • The PROMIS-10 Global Health (v1.2) form is available under Assessments in the patient chart.

  • The form includes 10 mandatory questions covering overall health domains.

  • All questions must be completed before submission.

  • Automatic calculation of Global Mental Health (GMH) and Global Physical Health (GPH) scores.

  • Scores are displayed in both raw and T-score formats.

  • Each question and score is mapped to the appropriate LOINC code.

  • Completed forms and scores are stored in the patient record for future review and export.

How It Works 

  • Navigate to the patient’s chart.

  • Select Assessments within Clinical Documentation.

  • Choose the PROMIS-10 Global Health (v1.2) form.

  • The form displays 10 required questions covering:

    • General health

    • Quality of life

    • Physical health

    • Mental health

    • Satisfaction with social relationships

    • Ability to perform usual roles and activities

    • Physical function

    • Emotional distress

    • Fatigue

    • Pain over the past 7 days

  • The user must answer all questions before saving.

  • If any question is unanswered, the system displays a message prompting completion of all fields.

  • Upon submission, the system automatically calculates GMH and GPH scores in both raw and T-score formats.

  • Scores and responses are saved in the patient record for review or export.
