Vitals Auto-Population and Percentile Handling in NYS School Health Examination Form

  • Vitals auto-population is now triggered when a Performed Date is entered or changed.

  • When the Performed Date is blank, all vitals fields remain empty for manual input.

  • The system retrieves vitals recorded on the exact Performed Date and overwrites all existing entries
  • If no vitals exist for that date, no auto-population occurs and manual entry is allowed.

  • Manual edits to vitals are preserved unless the Performed Date is changed again.

  • Removing the Performed Date after it has been set does not clear any vitals.

  • Percentile values automatically recalculate when:
    • The Performed Date is entered or changed, or

    • The BMI field is manually modified.

  • If BMI is changed while the Performed Date is empty, or if the percentile calculates to zero, all percentile checkboxes are cleared.

How It Works 

  • Navigate to Patient > Clinical > School Forms.

  • Open the NYS School Health Examination Form – Updated.

  • If the Performed Date is left blank, all vitals and percentile fields remain empty for manual entry.

  • When a Performed Date is entered or modified:

    • The system searches for vitals recorded on that exact date.

    • If found, the system auto-populates Height, Weight, BMI, Blood Pressure, Pulse, Respiration, and Percentiles.

    • All previously entered values—including manual edits—are overwritten.

    • If no matching vitals exist, auto-population does not occur.

  • Users may manually edit any vitals after auto-population; these edits persist unless the Performed Date changes again.

  • Removing the Performed Date does not alter or clear existing vitals.

  • Percentile checkboxes update when the Performed Date changes or when BMI is manually modified.

  • If BMI is changed while the Performed Date is blank or results in a percentile of zero, percentile checkboxes are cleared.
