What’s New
The Responsible Party section includes:
- Full Name, Relationship to Patient, Address, and Phone Number
- Phone numbers display in order of priority: Cell → Home → Office
The Emergency Contact section includes:
- Full Name, Relationship to Patient, Address, Phone Number (same priority order), and Email
- Multiple emergency contacts are shown with full details for each
If a field like phone, email, or address is missing, both the field and its label are omitted—no empty spaces or placeholders. The layout preserves left/right alignment for a clean appearance, even if one side has fewer details.
Behavior applies uniformly across both sections.
Why It’s Important
- Ensures only relevant and available information is shown
- Improved readability by removing blank labels or unused fields
