The form includes 10 mandatory questions covering overall health domains.

Automatic calculation of Global Mental Health (GMH) and Global Physical Health (GPH) scores.

Scores are displayed in both raw and T-score formats.

Each question and score is mapped to the appropriate LOINC code.

Completed forms and scores are stored in the patient record for future review and export.
How It Works

Navigate to the patient’s chart.

Select Assessments within Clinical Documentation.

Choose the PROMIS-10 Global Health (v1.2) form.

The form displays 10 required questions covering:

General health

Quality of life
-
Physical health
-
Mental health
-
Satisfaction with social relationships
-
Ability to perform usual roles and activities
-
Physical function
-
Emotional distress
-
Fatigue
-
Pain over the past 7 days

The user must answer all questions before saving.

If any question is unanswered, the system displays a message prompting completion of all fields.

Upon submission, the system automatically calculates GMH and GPH scores in both raw and T-score formats.

Scores and responses are saved in the patient record for review or export.
