PROMIS-29 Profile Form Added to Assessments Module

The PROMIS-29 Profile form is available in the Assessments module.

The form contains 29 mandatory questions grouped across multiple health domains.

Automatic calculation of raw scores for each domain during form completion.

Automatic generation and storage of scaled T-scores upon form submission.

Each question and score is mapped to its corresponding LOINC code.

Completed forms and scores are saved in the patient record for future review.
How It Works

Navigate to the patient chart and open the Assessments module.

Select the PROMIS-29 Profile form.

The form displays 29 required questions grouped by domain, including:

Physical Function
-
Anxiety
-
Depression
-
Fatigue
-
Sleep Disturbance
-
Ability to Participate in Social Roles and Activities

Pain Interference
-

The user must complete all questions before the form can be saved.

If any question is left unanswered, the system displays an alert requesting completion of all fields.

As responses are entered, raw scores are calculated automatically for each domain.

Upon saving the form, both raw scores and scaled T-scores are generated using a predefined scoring guide and stored in the patient record.

Scores and responses are displayed alongside the completed form for reference.
