All patient visits, regardless of patient age, with a diagnosis of cancer currently receiving chemotherapy or radiation therapy.

– NUMERATOR

Patient visits in which pain intensity is quantified.

– APPLICATION WORKFLOW

For Denominator 1:

All patient visits, regardless of patient age, with a diagnosis of cancer currently receiving chemotherapy during the measurement period qualify for the denominator.

  • To record a face to face/telehealth encounter, navigate to Patient > Provider Note > eSuperbill. The face to face/telehealth encounter must occur during the measurement period.

  • To record a cancer diagnosis, navigate to Patient > Provider Notes > Diagnoses. The patient should have been diagnosed with cancer at the time of the face to face/telehealth encounter. Click ‘Add’. Search for prostate cancer diagnosis and add the diagnosis. Then click ‘Save’. The diagnosis is added to the provider note.
Cancer Diagnosis
  • To record the ‘Chemotherapy Administration’ procedure performed, navigate to Patient > Provider Notes > Orders > Procedures. Click ‘Add’ and search for the procedure. Add the procedure details and click on ‘Save & Accept’/hover over ‘Sign & Accept’. If the user hover over ‘Sign & Accept’, then click ‘Complete’. The procedure is added to the provider note.
Chemotherapy Procedure

Two chemotherapy sessions should be performed and the dates of the first and second chemotherapy sessions must not overlap. First chemotherapy session should be performed 30 days or less on or before the day of face to face/telehealth encounter and second chemotherapy session should be performed 30 days or less on or after the day of face to face/telehealth encounter.

Note:

Please ensure that the Procedure is present in the system. For this, navigate to Settings > Billing > Procedure. If the procedure is not added then, click on the ‘Add Procedure’ button.

Here, enter the ‘Code’ and ‘Name’ of the Procedure. Once done, click on the ‘Save’ button.

For Denominator 2:

All patients, regardless of patient age, with a diagnosis of cancer currently receiving radiation therapy during the measurement period qualify for the denominator. The patient should have been diagnosed with cancer at the time, the radiation therapy procedure is performed.

  • To record a cancer diagnosis, navigate to Patient > Provider Notes > Diagnoses. The patient should have been diagnosed with cancer at the time of the radiation therapy. Click ‘Add’. Search for prostate cancer diagnosis and add the diagnosis details. Then click ‘Save’.
  • To record the ‘Radiation Treatment Management’ procedure performed, navigate to Patient > Provider Notes > Orders > Procedure. Click ‘Add’ and search for the procedure. Add the procedure details and click on ‘Save & Accept’/hover over ‘Sign & Accept’. If the user hover over ‘Sign & Accept’, then click ‘Complete’.

For Numerator 1:

  • Patients fulfilling the criteria for denominator 1 and who also had a pain assessment performed during the face to face/telehealth encounter are included in the numerator 1 of this measure. The result of the pain assessment must not be null. To document the pain assessment performed, use the workflow Patient > Clinical > Evaluations. Click ‘+ Evaluation’ and search for a pain assessment evaluation. From the ‘Status’ dropdown, select ‘Performed’, and add the result in the ‘Result’ field. Then click ‘Sign’ or ‘Save.
Evaluation

For Numerator 2:

  • Patients fulfilling the criteria for denominator 2 and who also had a pain assessment performed during the radiation therapy are included in the numerator 2 of this measure. The result of the pain assessment must not be null. To document the pain assessment performed, use the workflow Patient > Clinical > Evaluations. Click ‘+ Evaluation’ and search for a pain assessment evaluation. From the ‘Status’ dropdown, select ‘Performed’, and add the result in the ‘Result’ field. Then click ‘Sign’ or ‘Save.
