Weight Assessment and Counseling for Nutrition and Physical Activity for Children and Adolescents CMS155v10 Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported.

  • Percentage of patients with height, weight, and body mass index (BMI) percentile documentation
  • Percentage of patients with counseling for nutrition
  • Percentage of patients with counseling for physical activity
– DENOMINATOR

Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period.

– NUMERATOR – 1
Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period
– NUMERATOR –  2
Patients who had counseling for nutrition during the measurement period
– NUMERATOR – 3
Patients who had counseling for physical activity during the measurement period 
– DENOMINATOR EXCLUSIONS

  • Patients who have a diagnosis of pregnancy during the measurement period.
  • Patients whose hospice care overlaps the measurement period.

For Denominator:

All patients 3-17 years of age at the start of the measurement period, with at least one eligible encounter during the measurement period are included. To record an encounter, navigate to Patient > Provider Note > eSuperbill. Under the ‘Procedure- CPTs’ heading, enter the encounter code.

For Numerator  1:

All the Patients who had height, weight, and body mass index (BMI) percentile recorded during the measurement period are included in numerator 1. To record vitals, navigate to Patient > Provider Note > Vitals. Then, click on the ‘Add’ button. Enter the ‘Weight’ and ‘Height’ of the patient and the system automatically calculates the BMI. Once done click ‘Accept’.

For Numerator 2:

Patients who had counseling for nutrition during the measurement period are included in numerator 2. To record the intervention performed, navigate to Patient > Clinical > Evaluations. Click on ‘+ Evaluation’ and search for a counseling intervention. Select ‘Performed’ from the ‘Status’ drop-down, then click ‘Save’. Following codes may be used:

Evaluation

For Numerator  3:

Patients who had counseling for physical activity during a visit that occurs during the measurement period are included in numerator 3. To record the intervention performed, navigate to Patient > Clinical > Evaluations. Click on ‘+ Evaluation’ and search for a counseling intervention. Select ‘Performed’ from the ‘Status’ drop-down, then click ‘Save’.  Following codes may be used:

Evaluation

For Denominator Exclusions:

Patients who satisfy any one of the following criteria are excluded from the denominator.

  • Patients who have a diagnosis of pregnancy during the measurement period. To record diagnosis, navigate to Patient > Clinical > Diagnosis. Search for a diagnosis, add the diagnosis details, and click ‘Save’. Then click ‘Accept’ and the diagnosis is added to the provider note.
Diagnosis
  • Patients who were in hospice care during the measurement period. To document that patient is receiving hospice care outside of a hospital or long term care facility, navigate to Patient > Clinical > Evaluations. Click ‘+ Add’ and search for an intervention. Select ‘Order’ or ‘Performed’ from the ‘Status’ dropdown, then click ‘Sign’ or ‘Save’. Use the below mentioned codes to order or perform an intervention:
    • 385763009: Hospice Care (Regime/Therapy)
    • 385765002: Hospice Care Management (Procedure)
Evaluation
    • To document an encounter inpatient, navigate to Patient > Clinical > Evaluations
