Controlling High Blood Pressure CMS165v8 Percentage of patients 18-85 years of age who had a diagnosis of hypertension and whose blood pressure was adequately controlled (<140/90mmHg) during the measurement period
– DENOMINATOR

Patients 18-85 years of age who had a visit and diagnosis of essential hypertension overlapping the measurement period

– DENOMINATOR EXCLUSION

Patients with evidence of end stage renal disease (ESRD), dialysis or renal transplant before or during the measurement period. Also exclude patients with a diagnosis of pregnancy during the measurement period.

Exclude patients whose hospice care overlaps the measurement period.

Exclude patients 66 and older who are living long term in an institution for more than 90 days during the measurement period.

Exclude patients 66 and older with advanced illness and frailty because it is unlikely that patients will benefit from the services being measured.

– NUMERATOR

Patients whose blood pressure at the most recent visit is adequately controlled (systolic blood pressure < 140 mmHg and diastolic blood pressure < 90 mmHg) during the measurement period.

– APPLICATION WORKFLOW

For Denominator:

Patients 18-85 years of age who had a visit and diagnosis of essential hypertension overlapping the measurement period. To record diagnosis, navigate to Patient > Provider Note > Diagnosis.

Following workflow can be used to document diagnosis of essential hypertension: Patient > Provider Note > eSuperbill. The documentation of the first encounter takes place when one visit code gets attached with the patient’s eSuperbill/ charge.

For Numerator:

Patients whose follow-up blood pressure is adequately controlled(systolic blood pressure < 140 mmHg and diastolic blood pressure < 90 mmHg) during the measurement period are included in numerator. To record blood pressure, navigate to Patient > Clinical > Vitals.

For Denominator Exclusions:

If the patient met following criteria then he can no longer be a part of this measure.

  • An evidence of relevant end-stage renal disease(ESRD) diagnosis.
  • Documentation of dialysis or kidney transplant  before or during the measurement year.
  • Diagnosis of pregnancy during the measurement year.
  • Hospice care overlaps the measurement period.

To record an evaluation, go to Patient > Clinical > Evaluation.

User can set the threshold values of the Vitals:

  1. Click on the Settings icon from the top right corner of the Vitals tab.
  2. This opens the Add Vitals Ranges popup.
  3. Enter the values and click on the save button.
