For Denominator 1: All patient visits, regardless of patient age, who had an office, were diagnosed with cancer, and are receiving chemotherapy during the measurement period qualify for the denominator. To record procedures, navigate to Patient > Clinical > Orders & Results > Procedure For Denominator 2: All patients, regardless of patient age who had an office visit, were diagnosed with cancer, and are currently receiving radiation therapy qualify for this denominator. To document procedures, navigate to Patient > Clinical > Orders & Results > Procedure For Numerator: Patients from denominator who have patient assessments documented qualify for the numerator. To record this, navigate to Patient > Clinical > Evaluations.
