Patients who have an optic nerve head evaluation during one or more office visits within 12 months

– APPLICATION WORKFLOW

For Denominator:

All patients aged 18 years and older with a diagnosis of primary open-angle glaucoma. The system records office visits with the help of visit codes in the eSuperbill. To document that, navigate to Patient > Provider Notes > eSuperbill

For Numerator:

Patients who have any of the following components for optic nerve head evaluation performed during one or more office visits within 12 months:

  • Cup to Disc Ratio
  • Optic Disk Exam
