Falls: Screening for Future Fall Risk CMS139v8 Percentage of patients 65 years of age and older who were screened for future fall risk during the measurement period.
– DENOMINATOR

Patients of age 65 years and older with a visit during the measurement period.

– NUMERATOR

All patients that were screened for future fall risk at least once within the measurement period.

– DENOMINATOR EXCLUSIONS

Exclude patients whose hospice care overlaps the measurement period.

Exclude patients who were non-ambulatory at some point in the measurement period.

– APPLICATION WORKFLOW

For Denominator:
All patients who are 65 years and older with a visit during the measurement period are included in the denominator. The age is captured through patient demographics and the systems captures the visit when an office visit code or an annual wellness code is added on eSuperbill.

The documentation of the first encounter takes place when one visit code gets attached with the patient.

For Numerator:
If the patient is screened for Future Fall Risk at least once within the measurement period then he is included in the numerator of this measure. To record Falls Screening, navigate to Patient > Clinical > Evaluations.

For Denominator Exclusions:
Exclude patients whose hospice care overlaps measurement period or who were non-ambulatory at some point in the measurement period.
