– DENOMINATOR

Patients 65 years of age and older with a visit during the measurement period.

– DENOMINATOR EXCLUSION

Exclude patients whose hospice care overlaps the measurement period.

– NUMERATOR

Patients who have ever received a pneumococcal vaccination.

– APPLICATION WORKFLOW

For Denominator:

Patients 65 years of age and older with a visit during the measurement period are included in the denominator. To record a visit, navigate to Patient > Provider Note > eSuperbill. 

 

For Numerator:

Patients who have ever received any of the following are included in numerator:

  • Pneumococcal Vaccine (Patient > Clinical > Immunizations).
  • Procedure for Pneumococcal Vaccine Administered (Patient > Clinical > Orders and Results > Procedures).

These can also be added by navigating to Patient > Clinical > Evaluations.

For Denominator Exclusions:

Exclude patients who were in hospice care during the measurement year. To document this, navigate to Patient > Clinical > Evaluation.
