How to Access Survivorship Care Plan Templates for Oncology?

Survivorship Care Plan is a post-treatment care plan for cancer patients in the Provider Notes component of CureMD application. This form offers a brief summary of patient’s cancer staging and completed treatments. The project is a two-fold feature that can be used via Provider Notes.

Follow the workflow mentioned above to land on the ‘Note Templates’ screen.

Here users can access survivorship care plan templates for oncology practices and also download survivorship care plan templates from ‘Template Library’.

Once templates are downloaded, users can edit these forms from ‘Note Template’ settings.
