Pneumococcal Vaccination Status for Older Adults CMS147v8 Percentage of patients aged 6 months and older seen for
a visit between October 1 and March 31 who received an influenza
immunization OR who reported previous receipt of
an influenza immunization.
– DENOMINATOR

All patients aged 6 months and older seen for a visit during the measurement period and seen for a visit between October 1 and March 31.

– DENOMINATOR EXCEPTIONS
Documentation of medical reason(s) for not receiving influenza immunization (eg, patient allergy, other medical reasons)

Documentation of patient reason(s) for not receiving influenza immunization (eg, patient declined, other patient reasons)

Documentation of system reason(s) for not receiving influenza immunization (eg, vaccine not available, other system reasons)

– NUMERATOR
Patients who received an influenza immunization OR who reported previous receipt of an influenza immunization

– APPLICATION WORKFLOW

For Denominator:
All the patient which qualify for the initial population and have a documentation of a visit during the Flu Season (October,1st-Before Measurement Year till March,31st-During Measurement Year), are included in the denominator of this measure.For this measure, a patient visit during the Flu Season can have a documentation of any one of the following:

  1. Documentation of a performed Influenza encounter (Patient > Provider Note > eSuperbill)
  2. Documentation of a performed Hemodialysis procedure (Patient > Provider Note > eSuperbill)
  3. Documentation of a performed Peritoneal Dialysis procedure (Patient > Provider Note > eSuperbill)

 

For Numerator:
In order to get included in this measure a patient must have one of the following:

  1. Influenza vaccination (Patient > Clinical > Immunizations)
  2. An immunization recorded for influenza vaccine (Patient > Clinical > Immunizations > Add History Dose)
  3. A recorded history of a receipt of an influenza vaccine (Patient > Clinical > Evaluations)

If the patient has received an Immunization earlier, it gets recorded as history and can have one the following options in Practice/Facility.

  1. Home
  2. Work
  3. Surgery

For Denominator Exceptions:
Documentation of medical reason(s) for not receiving influenza immunization (eg, patient allergy, other medical reasons)

Documentation of patient reason(s) for not receiving influenza immunization (eg, patient declined, other patient reasons)

Documentation of system reason(s) for not receiving influenza immunization (eg, vaccine not available, other system reasons)
