– INITIAL POPULATION

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period.

– DENOMINATOR – 1

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period.

– NUMERATOR –  1

Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months.

– DENOMINATOR – 2

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period and identified as tobacco user.

– NUMERATOR – 2

Percentage of patients aged 18 years and older who were screened for tobacco use and identified as a tobacco user who received tobacco cessation intervention.

– DENOMINATOR –  3

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period

– NUMERATOR –  3

Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months AND who received tobacco cessation intervention if identified as a tobacco user.

– DENOMINATOR EXCEPTIONS 1

Documentation of medical reason(s) for not screening for tobacco use (eg, limited life expectancy, other medical reason).

– DENOMINATOR EXCEPTIONS 2

Documentation of medical reason(s) for not providing tobacco cessation intervention (eg, limited life expectancy, other medical reason).

– DENOMINATOR EXCEPTIONS 3

Documentation of medical reason(s) for not screening for tobacco use OR for not providing tobacco cessation intervention for patients identified as tobacco users (eg, limited life expectancy, other medical reason).

– APPLICATION WORKFLOW

For Denominator:

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period are included in the denominator.

To record the encounters, navigate to Patient > Provider Notes > Create eSuperbill. For instance, the visit codes i.e. G0438 and 99395 are used to record visits.

For Denominator 2:

All patients aged 18 years and older seen for at least two visits or at least one preventive visit during the measurement period and have smoking status saved as one of Current Smoker status qualify for this component.  To document this criteria, navigate to Patient > Clinical > History > Social

Tobacco Use Screening

Denominator 3:

It has the same criteria as Denominator 1.

For Numerator  1:

Patients who were screened for tobacco use at least once within 24 months and identified as non-smoker qualify for the numerator. To document this assessment along with its result, navigate to Patient > Clinical > History > Social.

For Numerator 2:

Patients who received Tobacco Cessation Intervention or Tobacco Cessation Pharmacotherapy qualifies for numerator 2.

To document Intervention, navigate to Patient > Provider Notes > Create eSuperbill OR Patient > Clinical > Evaluations.

To record relevant medications, navigate to Patient > Clinical > Medications.

Medications

For Numerator  3:

Numerator 3 includes any one of the following:

  • Patients who were screened for tobacco use and identified as a tobacco non user.
  • Patients who were screened for tobacco use, identified as a tobacco user and received Cessation Counseling for Tobacco User OR Tobacco Cessation Pharmacotherapy.

To document Intervention, navigate to Patient > Provider Notes > Create eSuperbill OR Patient > Clinical > Evaluations.

To document medication, navigate to Patient > Clinical > Medications.

Medications

Denominator Exceptions:

Documentation of medical reason(s) for not screening for tobacco use (e.g. limited life expectancy, other medical reason).

  • To record that “Tobacco Use Screening” assessment has not been performed, navigate to Patient > Clinical > Evaluations.
  • To add a diagnosis of “Limited Life Expectancy”, navigate to Patient > Clinical > Diagnosis.
