CMS122v6 – Diabetes: Hemoglobin A1c Poor Control CMS122v6 Percentage of patients 18-75 years of age with diabetes
who had hemoglobin A1c > 9.0% during the measurement period.
DENOMINATOR

All patients 18-75 years of age who meet the following criteria:

  1. An active diagnosis of diabetes
  2. A visit during the measurement period

– DENOMINATOR EXCLUSION:

Exclude patients who were in hospice care during the measurement year

NUMERATOR

Patients will be included in the numerator if any one of the following criteria is met:

  • HbA1c lab test was not performed for them.
  • The value for that test was not recorded.
  • The value was > 9%.
APPLICATION WORKFLOW

For Denominator:
The patient of age 18-75 with an active diagnosis of diabetes on the visit in the measurement year is considered for this measure. To record Diagnosis, navigate to Patient > Clinical > Diagnoses.

Visit – Diabetes

The system captures and calculates an encounter when a Visit code (E&M code) is documented in either eSuperbill or a charge.

DENOMINATOR EXCLUSION:

To exclude patients who were in hospice care during the measurement year, navigate to Patient > Clinical > Evaluation. 

Denominator Exclusion

For Numerator:

Patients will be included in the denominator if any one of the following criteria is met:

  • HbA1c lab test was not performed for them.
  • The value for that test was not recorded.
  • The value was > 9%.

To record HbA1c level, navigate to Patient > Clinical > Orders and Results > Labs.
