Patients of age 66 years and above, who had a visit during the measurement period. – NUMERATOR 1 Patients with an order for at least one high-risk medication during the measurement period. – NUMERATOR 2 Patients with an order for at least two different high-risk medications during the measurement period. – Denominator Exclusions Exclude patients who were in hospice care during the measurement year |

– APPLICATION WORKFLOW
For Denominator: For Numerator 1: On a visit during the measurement year patient with an order for at least one high-risk medication that is to be taken for more than 90 days is included in the numerator. In order to record this, navigate to Patient > Clinical > Medications. For Numerator 2: – Denominator Exclusions Exclude patients who were in hospice care during the measurement year. To document this, navigate to Patient > Clinical > Evaluation |
