– DENOMINATOR

Women 16 to 24 years of age who are sexually active and who had a visit in the measurement period.

– NUMERATOR

Women with at least one chlamydia test during the measurement period.

-DENOMINATOR EXCLUSIONS

Women who are only eligible for the initial population due to a pregnancy test and who had an x-ray or an order for a specified medication within 7 days of the pregnancy test.
Exclude patients who were in hospice care during the measurement year.

– APPLICATION WORKFLOW

For Denominator:
Women 16 to 24 years of age who are sexually active and who had a visit in the measurement period are included in the denominator for this measure. To record visit and diagnosis, navigate to Patient > Provider Note > Diagnosis.

Diagnosis

For Numerator:

The documentation of the first encounter takes place when one visit code is documented on the patient’s eSuperbill/ charge.

Women with at least one chlamydia test during the measurement period are included in the numerator. To record lab test, navigate to Patient > Clinical > Orders and Results > Lab.
