For Denominator:
Patients 5 to 17 years of age with a diagnosis of diabetes where the diagnosis overlaps the most recent visit during the measurement period are included in the denominator for this measure. To record diagnosis, navigate to Patient > Provider Note > Diagnosis.

Diagnosis

The documentation of the first encounter takes place when one visit code is documented on the patient’s eSuperbill/ charge.

For Numerator:
Patients with documentation of date and result for a HbA1c test during the measurement period are included in the numerator. To record lab result, navigate to Patient > Clinical > Orders and Results > Lab.
