CMS165v5 – Controlling High Blood Pressure

Patients 18-85 years of age who had a diagnosis of essential hypertension within the first six months of the measurement period or any time prior to the measurement period.

– DENOMINATOR EXCLUSION

All patients with:

1. evidence of end-stage renal disease (ESRD) before or during the measurement year
2. Documentation of dialysis or kidney transplant
3. Diagnosis of pregnancy during the measurement year

– NUMERATOR

Patients whose follow-up blood pressure is:

Adequately controlled(systolic blood pressure < 140 mmHg and diastolic blood pressure < 90 mmHg) during the measurement period.

For Denominator:

Patients 18-85 years of age who had a diagnosis of essential hypertension within the first six months of the measurement period or any time prior to the measurement period. To record diagnosis, navigate to Patient > Provider Note > Diagnosis.

For Numerator:

Patients whose follow-up blood pressure is adequately controlled(systolic blood pressure < 140 mmHg and diastolic blood pressure < 90 mmHg) during the measurement period are included in numerator. To record blood pressure, navigate to ”’Patient > Clinical > Vitals”’

Denomenator Exclusions:

If the patient has,

• An evidence of relevant end-stage renal disease(ESRD) diagnosis before or during the measurement year
• Documentation of dialysis or kidney transplant
• Diagnosis of pregnancy during the measurement year

then the patient can no longer be a part of this measure.

You can set the threshold values of the Vitals:

1. Click on the Settings icon from the top right corner of the Vitals tab.
2. This opens the Add Vitals Ranges popup.
3. Enter the values and click on the save button.
