Diabetes: Foot Exam CMS123v5 The percentage of patients 18-75 years of age with diabetes
(type 1 and type 2) who received a foot exam
(visual inspection and sensory exam with mono filament and a pulse exam)
during the measurement year
DENOMINATOR

Patients 18-75 years of age with diabetes with a visit during the measurement period are included in the denominator.
Patients between 18 years and 74 years inclusive. You may add or change the age from any patient’s profile.
Patients Diagnosed with Diabetes. This can be documented from Diagnosis module in any patient’s profile.

DENOMINATOR EXCLUSION

Patients who have had either a bilateral amputation above or below the knee, or both a left and right amputation above or below the knee before or during the measurement period. Exclude patients who were in hospice care during the measurement year.

NUMERATOR

Following physical exams can be added in Evaluation module to count for this measure:

  1. Pedal pulse taking (procedure)
  2. Monofilament foot sensation test (procedure)
  3. Diabetic foot examination (regime/therapy)

Denominator Exclusion

Patients who have had either a bilateral amputation above or below the knee, or both a left and right amputation above or below the knee before or during the measurement period.

APPLICATION WORKFLOW

For Denominator:
Patients diagnosed with diabetes are included in denominator.

For Numerator:
Patients who received  one of the following visual, pulse and sensory foot examinations during the measurement period are included in numerator.

  • Visual Exam of Foot
  • Sensory Exam of Foot
  • Pulse Exam of Foot

Diabetes Exam

Denominator Exclusion

Patients who have had either a bilateral amputation above or below the knee, or both a left and right amputation above or below the knee before or during the measurement period. To document this encounter, navigate to Patient > Clinical > Provider Notes. 

Denominator Exclusion

If the diagnosis, Q72.20 with comments: ‘Right’ is attached with the Provider Note, that patient goes to exclusion.
If Q72.21 with the comments: ‘Left’ is saved and attached with the provider note, it goes to exclusion as well.
