What is EC4 – NARR Form?
Patient > Billing > Quick Charge
On EC4-NARR form, following fields are auto-populated:
- WCB case number
- Carrier Case number
- WCB Auth number
- WCB Rating code
The link on the bottom right labeled Review Demographics and Billing Address shows the Provider Profile of the Rendering Provider on the associated charge in a popup.
