What are ABN Forms?

An Advance Beneficiary Notice (ABN) is also known as a waiver of liability. It is a notice that a provider gives to their patient before the patient receives any service.
Based on Medicare Coverage Rules a provider may have their patients fill out and sign an ABN form if there is reason to believe Medicare might not pay for the services being rendered.The ABN allows the patient to decide whether to receive the service in question and to accept financial responsibility for the service (pay for the service out-of-pocket) if Medicare denies payment.

To create an ABN form, follow the workflow above and click on theĀ ‘Add ABN’.

Once the page loads, the provider has to select the Insurance plan, fill out the description and mention the services that he will render, the cost per services being rendered and the reason why he believes the insurance may not cover the charges. The patient is given the form to read and is asked to select from one of the given options.
